The Social Security Administration form used to request an IRMAA recalculation after a major life-changing event. Eight events qualify: marriage, divorce, death of a spouse, work stoppage, work reduction, loss of income-producing property, loss of pension income, and employer settlement payment. Submit with documentation (pay stubs, decree, death certificate, employer letter) and SSA can use a more recent year's MAGI. Approval typically takes 30 to 60 days. Appeals on denial follow the standard SSA appeals process.
